What Does My Thyroid Profile Test Report Indicate?

default
Posted on Fri, 26 May 2017
Question: Respected Madam/Sir,

My wife aged 36 (house wife) had tested her thyroid profile in sept. 2014. Please have look at below for the result.
T3 - 2.02 nmol/L. T4 - 100.57 nmol/L and TSH - 6.89 mIU/ml.
Looking at TSH, Endocrinologist advised to take THYRONORM 25 mg daily one tablet.
This tablet she has been taking till date. Day before yesterday(5 May, 2017), again she tested her Thyroid and result is at below :
T3 - 1.27 ng/mL. T4 - 9.50 ug/dL and TSH - 3.86 uIU/mL.
She visited another Endocrinologist and doctor advised that medicine suggested by earlier doctor was wrong and she has to stop taking medicine further. Doctor added, it is not required to take medicine at TSH level- 6.89 mIU/ml, which was found in year 2014.

Q 1. Now she is confused and not sure which endocrinologist was correct. Please advice and confirm whether to stop or to take medicine (THYRONORM 25 mg).

Q 2. Please advise that will it help her to reduce her weight if she start running on Treadmill daily. She weight around 70 KG ? If really it helps, then she can buy a Treadmill and start running on it.
As she has Cervical Rib on Neck and also suffering from Low Back Pain due to loss of disc space between L3 and L4 vertebrae. So is it safer for her running on Treadmill daily ? Please advise.

Many Thanks, XXXXXXX

Brief Answer:
About subclinical hypothyroidism:

Detailed Answer:
Hello and thank you for asking!
I can understand your concern regarding to your wife's health!
When TSH level is high (4-10 mUI/Ml) and FT4, FT3 are normal, we describe it as subclinical hypothyroidism and begin treatment with levothyroxine if she has clinical symptoms (weight gain, dry skin, constipation, irregular menstrual cycle) or if she plans a pregnancy.
So, in this case, i find it logical to start with a low dose of levothyroxine, that has brought the TSH back to normal levels. I would suggest to continue with the same dose and check the TSH after 6 months.
Regarding to weight loss, diet and physical activity would be recommended. Probably a fast walk, rather than running would be more convenient as she has the loss of space between discs. Anyway the physician in charge will give more exact information regarding to the kind of the activity she needs (depending on the severity of the discopathy).
Hope the information will help.
Kind regards,
Dr.Mirjeta

Q. Based on your advice/information I understand that it was correct decision for her starting medicine since 2014 (at TSH level 6.89 mIU/mL ) and to be continued (at TSH level 3.86 uIU/mL) for next 6 months. Please confirm.

Just for your kind information, she has clinical symptom like kind of gaining weight in 2014 and now as well.

Brief Answer:
Keep monitoring the TSH every 6 months;

Detailed Answer:
Welcome back,
That is why she needs to be treated.
It is a low dose for the moment but keep checking the TSH every 6 months to find out the time when the dose should be increased.
Meanwhile the diet will help to loose weight.
Do not hesitate to ask if you have still queries.
Best wishes,
Dr.Mirjeta
