Excessive Body Hair Growth. High Testosterone. Recommended Hysterectomy To Find Lesions In Ovaries. Suggestions?

A 58 yr old woman with excessive body hair growth seeks medical attention. Testosterone levels are highly elevated(147) and she also found to be hypothyroidism(TSH= 10). After doing multiple lab test all her adrenal blood work came back normal (CT scan of the adrenals is also normal). Vaginal ultrasound looks completely normal( no abnormalities shown in ovaries). Dr. Recommends total hysterectomy( including ovaries and the tubes) to study ovaries to find out any possible lesions in the ovaries. What is your perspective? Thank you

Thanks for your question.
I would like to find out other causes of raised serum testosterone first, like exogenous intake of testosterone; may be excessive or unsupervised use of some hormonal products. So, drug history should be in detail as many herbal remedies may create such pictures. After that I would like to involve an endocrinologist to treat her thyroid deficiency & to give some expert opinion. Tests like doppler ultrasound, CT scan, MRI may provide helpful clues to rule out any pathology from ovaries. I think the doctors should look for other rare sites for hormone producing tumors ( carcinoid tumors). Her history & systemic review is likely to be extremely helpful. I don't think jumping to such a major surgery without a former diagnosis is justifiable.
