How Can Blood Sugar Fluctuations Be Controlled Better?

Hello doctor, I am Bharathi rao, aged 55, height 1.62M. I have been diabetic since 3 years and on insulin ince 2 yrs. Doctors diagnosed it as BRITTLE DIABETES. Readings are highly fluctuating. Morning if I check it willl be 160 (fasting) and after 2 hrs it rises to 250 and above. It will nver be on normal . Daily I am having 3 pricks. Morning (1) Lantus and Humolog 50:50 and night Humalog. Lantus 22 units and Humalog 35 units and in the night Humalog 22 units. This is in addition to GLYCOMET 500mg (thee times a day) Sugar levels may go to 400 (high side) or less than 50. I would like to know whether I can control the sugar without high ups and downs. I am having Thyroid (under control) andBP also. My parents are non-diabetic. But myself, my sister and brother are diabetic. Is there any medicine which can control the fluctuations in the sugar levels. regards, bharathirao.

Good day. Your insulin regimen seems to be a bit strange. I wouldn't normally combine Lantus with Humalog mix. It is okay to use Lantus with Humalog ( not Humalog mix). As you have so much fluctuations, you may require a test called as CGMS ( continuous glucose monitoring system) for 3 days. This is extremely useful in this kind of situations. I would say, better to consult an endocrinologist who can help you further. Few of my patients with brittle diabetes do very well with Insulin pump as well
