I Have Symmetrical Bilateral Pain In Buttocks And Lower Back
27yo female; symmetrical bilateral pain in buttocks and lower back, sometimes involves both great throcanters too. Gets better with movement, worse at night when lie in bed. Think it is sacroiliitis. Would like to know if there are other possibilities, and even how to diagnose sacroiliitis avoiding x-rays exposing techniques: know of MRI , is there any blood test?

The back pain has different causes. Osteoarthritis, Lumbar Disc Hernation, Rheumtoid Arthtris and Systemic Lupus Erythemotosis like connective tissues disease can involve back.
I recommend , ESR, Rhematoid Factor, MRI of the back. X ray has limited use.

See a pelvic floor physiotherapist. Pelvic floor pain manifests in different area from waist to hips.

first of all you should consult a rheumatologist, sice it look like an auto-immune disorder, so the examination of the patient is very important to rule out other condition like PIVD, Osteonecrosis of femoral head, etc. Than there is role of investigations like blood parameters, X-ray, & MRI Scan
