My 9 month old has been crying and moaning for 4 hours. He passed a large amount of large hard cylindrical stool 3 hours ago. He has a low grade fever 101.7. He is refusing formula -- he has had 14 oz today, but normally would have had 22 by now. He has been sick a lot this winter -- repeated bronchiolitis, strep, croup, norovirus, but has NEVER been this upset. He's unable to nap even in my arms.
My assessment is that he has something that needs urgent attention in the ER. I would not call a temperature of 101.7 a "low-grade" fever. It means that there is significant fever, and the least that is needed is to take him to the ER for blood counts, a culture and clinical assessment to rule out anything serious.
