Is There A Humalog Insulin Sliding Scale Available?

I am looking for a chart of the sliding scle forhumalog insulin. 69 years old.245lbs diabetic on insulin type 2. I want ascale to tell howmuch nsulin i take when my bloosugar is above normal and how much to increase the humalog as the numers increase.
