Pregnant, Spotting, Positive UPT, No Gestational Sac, TVS Suggested. Reason?
I am a 27 y.o lady. Recently i was confirmed pregnant as my Upt done 3x and all positive. I got spotting from 23 to 25th of August (which i thought the menses time) then on the 25 i do my 1st upt. +ve. Due to spotting i went to the emergency at hospital. Upt done one more time. Positive. Estimated gestation about 6w 3d. Scan done but no gestational sac seen. The doctors said maybe due the empty bladder. Then was prescribed with Duphaston 10mg TDS for 2/52. Spotting stop on 26 August. Last color about brownish. No abd pain. Yesterday, 2nd Sep i went to GP to do the TAS. currently at 7w 4d. Upt still +ve. But yet still no gestational sac seen. But the doc said I got bulky uterus.. can u tell me whats really going on. Coz as what i've read before. The gest sac could be seen in the 5w of pregnancy.. i am quite worried. And was suggested to do the TVS. TQ
UPT positive is the presence of HCG in the urine which is an accurate test for pregnancy. There might be difference in last menstrual period dates as documented by you and actual dates considering little bleeding.
You must go for TVS now and confirm the presence of gestational sac and/ or decidual reaction. This happens when the uterus is ready to harbour a pregnancy.
Hi, In the current scenario, trans-vaginal sonogram coupled with serial serum beta-hCG titres will give a clarity about the state of your pregnancy. It is sometimes difficult to see the gestation sac clearly in an abdominal sonogram in very early pregnancy. Doubling hCG titres on alternate days along with identification of the sac in sonogram indicate good prognosis. Meanwhile avoid all strenuous activity including travel and sexual intercourse and take adequate bed rest. Hope your query is clarified. Good luck.
Hello Thanks for your query. At 7weeks 4 days, if no gestational sac is seen, the pregnancy is mostly non viable. A transvaginal scan is MUST before this diagnosis is considered. Transabdominal scan would not yield good details or diagnosis. Please go for TVS, and if no gestational sac is still seen, please consider termination of the remaining products of conception. Hi, I can understand your concern. Repeated Upts being +ve, but no formation of embryo.Such non embrionic pregnancy is called as blighted ovum . the pregnancy is ultimately aborted. Every alternate abortion in first 3 months of pregnancy is blighted ovum. Though it is a well known phenomenon, a normal pregnancy must be excluded before declaring the diagnosis.TVS sonography gives more accurate view. therefore the results are more reliable.That's why it's planned for you. This is a very concerning story for an ectopic pregnancy (a pregnancy in your tube). The earliest that a pregnancy test will turn positive is about 4 weeks after the start of your last period. Lets assume that was on about 8/25. By now (3 weeks later) there should absolutely be a sac in the uterus, and if there is not, then you have either miscarried or you have an ectopic pregnancy. You need an HCG level drawn NOW and an ultrasound to sort this out. Ectopic pregnancies are life-threatening - there is not time wait sorting this out.
