uncontrolled Diabetes mellitus can lead to the development of a number of microvascular complications, and nephropathy, is one of them. It is recommended that you get your urine for spot ACR or 24 hrs urine for protein estimation done. If there is microalbuminuria/macroalbuminuria (albumin in urine), you need to start on ACE inhibitor/ ARB group of drugs (since your potassium is normal). This drug is going to help you out both for your Blood pressure and nephropathy. A USG of the
kidney will reveal whether any chronic changes have developed in the kidney.
Presently, you are on
Insulin 10 and 8 units (the type of insulin is not mentioned, I assume it to be of the premixed type). HbAic can indicate the control of your blood sugar over 3 months.
Dietary modifications with protein restriction (0.6-0.8gm/kg body weight) will help.
