Suggest Treatment For Diabetic Ketoacidosis With Improper Insulin Intake

I believe my friend has been suffering from DKA for possibly a week, maybe longer... when sent to the Doctor, she lied (yes, I don t understand this either) and said she was taking her Insulin 3 times a day, rather than 1. She has a hard time keeping food down and has lost weight. Often I will talk to her and she will be laying down, within a few minutes she is asleep... no matter how much sleep she has had. She is going to college out of state, and I am not there in person - so I cannot force her to go to the doctor - but if its serious, I can get ahold of her relatives and or medical emergency staff.. I am hoping this will clarify the level of concern I should communicate to her. She is 21, Female. Type 1 diabetes.

Dear friend
i can understand your concern
in my opinion your friend might be suffering from uncontrolled diabetes
the only thing that need to be done in type 2 diabetes without fail is taking insulin.
i would like to tell some facts about DKA
1.DKA is a medical emergency
2.in DKA pt suffers problems mostly due to dehydration and electrolyte imbalance.
3.insulin need to be taken at least once a day to prevent DKA but in type 1 diabetes its very difficult to get control with only single time insulin.

the best treatment for her is take her to a diabetes educator and advise to go with some counselling, hope she will understand the seriousness of condition
Hope I was able to help you.
If you have any further question feel free to write me back. I will be happy to help.
Take care.
Best Regards.

Internal Medicine Specialist Dr. Noble Zachariah's  Response
Hello,

i appreciate your concern for your friend.
As you rightly suspected, she is quite ill . She would require to be admitted and properly managed. Along with improving her diabetes control and correcting the probable ketoacidosis, her psychological assessment also has to be done. The reason why she is avoiding treatment should be addressed and properly managed by the psychiatry team. She would look after herself well and should be in a position to control her diabetes as per the instructions of the treating doctor. This alone can give her good health and good quality life in the long run.
Wish your friend a quick recovery
