Hello my fiance was diagnosed with malaria over a week ago.. since then she has been in and out of the hospital, suffering from bouts of fever, body pain and unconsciousness, often lethargic... recently she has been told that she is having brain fever... could someone advice on the best treatment for her. PLEASE
Please hospitalize her immediately. The recommended treatment after admission can be as follows:
1. IV Quinine 10mg/kg as an infusion over 4-8 hours thrice daily for 7 days. If she is resistant to quinine, please go for artesunate 2mg/kg IM on day 1 and 80 mg (2mg/kg) IM on days 2 to 5. This should clear her parasitemia faster than other anti malarial drugs.
2. Supportive treatment to avoid the following will be required: > Hypoglycemia > Hemoglobinuria > Jaundice > Hypotesion
Although cerebral edema is rare in adults, please watch out for any increase in intracranial pressure.
