Treatment For Bumps On Arms And Rosacea On Face

Hi doctor, I am 17, 5 foot 3 inches, 130 lbs. I have skin problems such as little bumps on my arms (I think it's KP) and possibly eczema or rosacea on my face, but I'm not sure. Since I was little my cheeks have always been red, but now they've gotten worse. They are red and soft on the cheek and as it get closer to the hairline it becomes redder and bumpy and rough. This extends to my jawline but does not reach my chin, nose or forehead. What should I do to make the redness go away and my cheeks softer?
