Bleeding during Mirena insertion is commonly due to minor irritation, some experience bleeding and dizziness during insertion but the symptoms should go away in 30-40 minutes. Cramping and spotting after Mirena insertion is common. You can take pain relieving medicines for relief.
If the bleeding is continuing and is heavy, you are advised to clinically examined by a gynecologist to rule out perforation, internal injury, etc. You will be treated / managed accordingly. Other side effects of Mirena are severe allergic condition, mispositioning, severe pelvic pain, sepsis, increased or decreased bleeding, no periods, ovarian cyst, etc.
Take care. Hope I have answered your question. Let me know if I can assist you further.
