Hello there, I am dr.milan an infectious disease specialist answering your question. Hope i have given appropriate guidance to you. There are three most common cause Malaria, Urinary tract infection and Lower respiratory tract infection. TO rule out these you can correlate history which system is infected. If it is malaria it may be relapse of malaria because he had malaria when he is in previous country and not treated properly it may relapse any time after any long time. So first cause is this. Other cause is ruleout by history and than treat it after doing culture test and follow the sensitivity report. if you have any query you can consult me anytime. Give me star rating, helpful vote & thank you according to your satisfaction level. Thanking you.
