gout , so at this time according to your query you may have gout once again , so get in blood examination for uric acid level .

2nd reason of your index finger swelling may be onset of
rheumatoid arthritis , so get in blood examination for rheumatic factor for confirmation of rheumatoid arthritis .
3rd reason of this swelling may be
osteoarthritis , however OA usually effects larger joints as knee ,but in few cases smaller joints may involve.
So in my opinion first of all consult an orthopedic surgeon and get his opinion.
