What Does Rt Middle Cerebral Artery Narrowing And Lt Cerebellar Artery Absent Indicate?
10 yr old girl presenting with generalized oedema ,proteinuria, fist,followed by hemiparesis. CT(head) show cerebitis and renal biopsy show FSGS. We treated with Iv cycliphosphamide pulse therapy follow by maintenance . 1 yr after this she present with heaviness of left side of limds and pANCA positive. ctangiogram show rt middle cerebral artery narrowing and lt cerebellum artery absent and mod degree of atrophy (lt cerebral) and severe atrophy of lt cerebellum . proteinuria resolved and no other skin lesion. What is next treatment?
1. As history child have neurological symptoms earlier with brain infection and one-sided weakness had treatment. 2.The present scan reveals there is a diffuse narrowing of the Right MCA this could be after infective process there is changes in the artery caused the changes in the vessel. 3.Absent cerebellar artery could be complete narrowing or congenital defect ( at atrophy features are also seen) 4.In this case probably an interventional procedure to dilate the vessels which are occluded will be done in presence of specialist to do that and baby he alt should support. 5.Suggest to see and discuss with complete old reports with neurosurgeon that will give you appropriate suggestion Good luck.
