What Is The Treatment For Itchy Painful Breast?

I am 54 years old and I have a breast that has a dark nipple. That is itchy and I have had it for awhile. Now I have a shooting pain when I turn over in bed and run. The pain comes from my nipple and shoots to the back of my breast. Should I be concerned.
