The best way to get the doctors you need is through your primary health care physician. He constantly evaluates you and make the decision on what your various health needs are and which of the physicians can help you most. This is more important when you need several specialist to attain to you and the need someone to coordinate and integrate all of this care for you. You would most likely need the services of a diabetologist, neurosurgeon, neurologist, etc and there is no single physician who specializes in all this.
