Why Are My Sugar Levels High Even After Taking Insulin ?

My father is a diabetic for 25 years and now he takes 18 units of Insulin in the morning. However, his sugar levels are around 300-350 post lunch. He had undergone a CABG surgery and was advised 18 units of Insulin during the morning after discharge. Kindly advise why the sugar levels are so high. Please let me know solutions for the same as I am a bit alarmed.

Was he on oral tablets before surgery, which were stopped before shifting to Insulin?

If yes, the tablets should be restarted after you doctor's nod.

If no, Insulin dose, type and timing needs to be adjusted. Some oral tablets in addition to it might help too.
