So this virtually rules out any insect bite, infection or allergic reaction. Bilateral painful toe swelling is a characteristic feature in gout, venous thrombosis due to blood clot or arthritis. Since you are having arthritis, so this is an important possibility as it can be due to worsening of arthritis. But at the same time, gouty arthritis needs to be ruled out. In gout, uric acid crystals deposits in the joints causing swelling, pain and tenderness. Big toe is commonly affected.
