Suffer From Low B12, Vitamin D, Hypothyroidism, Fatty Liver. Treatment?

I live in a small town and have been dealing with low b12 despite the shots which I take monthly, lowvitamin d, hypothyroidism and now I am fighting thyrodittis. I also suffer from fatty liver and swelling.... is there any illnesses that causes all of theses issues.... I am only 37 and I am the mother of 4 and was normally full of energy.
