Suggest Treatment For Incompetent Sapheno Femoral Junction With Varicose Veins
I am a 41 year old female with 2 children. Wt. 70KG. Ht. 5.2'. I have varicose veins in both my legs. Recently, I had consulted a Vascular surgeon who had advised me to have colour doppler test. I have reproduced the test report below. Please advise whether it requires any surgery and how soon.
* Sapheno femoral junctions are incompetent bilatrally, more on right side with varicose veins along long saphenous system
* Blow outs are noted from long saphenous vein in the medical aspect of upper and mid thigh on right side.
* Impression - Bilateral sapheno femoral junction incompetence and incompetent perforators with varicose veins
No evidence of deep vein thrombosis
* Sapheno femoral junctions are incompetent bilatrally, more on right side with varicose veins along long saphenous system

So far as treatment point of view is concerned for varicose veins , depends largely on the symptomatology and concern of the patient, correlated with the clinical evaluation and Doppler findings.
I am discussing both ways of management :
1. Conservative management (Non-operative) :
-- suggested when it`s not much symptomatic, much troublesome .
-- advise patient to keep both lower limbs elevated above 3-4 pillows when
resting.
-- keep mobilisation with compression stockings.
-- avoid smoking / alcohol beverages.
-- medicines as and when required for pain only.
-- control of BP , Blood Sugar, Cholestrol level under control.
2.Surgical management
-- when symptoms are more troublesome, more concerning , non-operative
measures not working / patient demands / surgeon decides that it`s
mandatory.
-- various procedures ranging from laser / coagulation therapy / vivid
operative procedures depending on the Vascular Surgeon`s /
Interventional Radiologist`s concern.
